Before you nominate - please read our strict criteria

Before scrolling down to request consideration for a Nominee to recieve an instrument, please read the below prior to applying, as we will require proof in all instances (sadly there are people who are less than honest or genuine in this world)

 We only supply instruments to those who meet one or more of the following criteria:

• Have an ongoing Cognitive or Mental Health concern which is documented **
• Have a diagnosed Physical Disability which is documented **
• Children suffering with Life Limiting and/or Dehabilitating Conditions and Disease which are documented **
• Have endured Major Trauma - This will be judged on a case by case basis and refers to children who have had trauma happen to them or around them (loss of a Parent, Abuse, Orphaned or Taken in to Care are all good examples of the Level of Trauma to refer too as a guide)

** Please note - Where possible & to strengthen your request we will require proof to be uploaded along with your initial request below - Forms of proof we accept are:

• Medical letters stating diagnosis
• Benefits letter stating childs name and diagnosis (DLA Benefit)
• ECHP summary
• Therapy assessments / reports
• School-based information
• Other official letters demonstrating may be considered if you do not have any of the above, however these may not guarentee acceptance if they fail to clearly state 

We do understand the concern around sharing the information above, but we treat all information in strict confidence and in accordance with our Privacy Policy. We will not retain or share any informaiton provided - it is purely to verify the application.

We want to ensure the right chidren get our donations so unfortunately without any of the above we are unable to supply an instrument and your nomination refused without notice.

Incredibly we have seen some of our instruments we have donated find their way on to sales sites - this is strictly forbidden and completely unfair when a child could of made great use of any instrument that is shamefully sold on - we are always willing to take back instruments should it not work out for the child & will re-home free of charge !
